Amenorrhea can be caused by any number of changes in the organs, glands, and hormones involved in menstruation.  Stress due to internal or situational concerns can cause secondary amenorrhea, because stress interferes with the brain&#8217;s control (through hormones) of the ovaries.  Amenorrhea may be classified as primary or secondary.  primary amenorrhea - from the beginning and usually lifelong; menstruation never begins at puberty.  Primary amenorrhea is defined as the failure of menses to occur by age 16 years.  Secondary amenorrhea - due to some physical cause and usually of later onset; a condition in which menstrual periods which were at one time normal and regular become increasing abnormal and irregular or absent.  Secondary amenorrhea is defined as the cessation of menses once they have begun.  This problem is seen in about 1% of women of reproductive age.  Amenorrhea occurs if the hypothalamus and pituitary fail to provide appropriate gonadotropin stimulation to the ovary, resulting in inadequate production of estradiol or in failure of ovulation and progesterone production.  Amenorrhea can also occur if the ovaries fail to produce adequate amounts of estradiol despite normal and appropriate gonadotropin stimulation by the hypothalamus and pituitary.  Chronic conditions (eg, starvation, excessive exercise, depression, psychological stress, marijuana use, Crohn disease, cystic fibrosis, sickle cell disease, thalassemia major, HIV infection, renal disease, thyroid disease, diabetes mellitus, anorexia nervosa) &#13;</p>
<p>Physiologic states of amenorrhoea are seen during pregnancy and lactation (breastfeeding).  The hypothalamus is the initiator of the follicular phase.  The gonadotropin-releasing hormone (GnRH) pump located in the hypothalamus releases GnRH in a pulsatile fashion into the portal vessel system surrounding the anterior pituitary gland.  GnRH interacts with the anterior pituitary gland to release follicle-stimulating hormone (FSH) in the follicular phase.  FSH is secreted into the circulation and interacts with the granulosa cells surrounding the developing oocytes.  As levels of progesterone, estradiol, and inhibin decline 2-3 days before menses, the hypothalamus begins to release higher levels of FSH, which recruits oocytes for the next menstrual cycle.  As FSH increases during the early portion of the follicular phase, it interacts with granulosa cells to stimulate the aromatization of androgens into estradiol.  Early in the follicular phase, both estradiol and FSH increase the FSH-receptor content of the developing follicles.  Over the next several days, the steady increase of estradiol (E2) levels exerts a progressively greater suppressive influence on pituitary FSH release.  Only one selected lead follicle, with the largest reservoir of estrogen, can withstand the declining FSH environment.  The remaining oocytes that initially were recruited with the lead follicle undergo atresia.  Immediately prior to ovulation, the combination of E2 and FSH leads to the production of luteinizing-hormone (LH) receptors on the granulosa cells surrounding the lead follicle. &#13;</p>
<p>Hormonal contraceptives that contain only progestogen like the oral contraceptive  Circulating estradiol stimulates growth of the endometrium.  Progesterone, produced by the corpus luteum formed after ovulation, transforms proliferating endometrium into secretory endometrium.  During the late follicular phase, estrogen positively influences LH secretion, instead of suppressing pituitary LH secretion as it does early in the follicular phase.  To have this positive effect, the E2 level must achieve a sustained elevation for several days.  The LH surge promotes maturation of the dominant oocyte, the release of the oocyte and then the luteinization of the granulosa cells and the surrounding theca cells of the dominant follicle resulting in progesterone production.  The appropriate level of progesterone arising from the maturing dominant follicle contributes to the precise timing of the mid-cycle surge of LH.  E2 promotes uterine endometrial gland growth, which allows for future implantation.  Other signs or symptoms along with the absence of periods, such as milky nipple discharge, headache, vision changes, or excessive hair growth on your face and torso (hirsutism). &#13;</p>
<p>Treatments of Amenorrhea based on the condition.  Medical care needs are defined by the etiology of the menstrual cycle disturbance and the desires of the patient.  Progesterone supplements (hormone treatment).  Gonadotropin therapy or the use of pulsatile GnRH therapy is required to induce ovulation for patients with infertility whose underlying pathology cannot be reversed.  Dopamine agonists are effective in treating hyperprolactinemia.  Oral contraceptives (ovulation inhibitors).  Dietary modifications (to include increased caloric and fat intake).  Hormone replacement therapy is required to maintain bone density in patients whose underlying pathology cannot be reversed to restore normal endocrine function.  In most cases, physicians will induce menstruation in non-pregnant females who have missed two or more consecutive menstrual periods, because of the danger posed to the uterus if the non-fertilized egg and endometrium lining are not expelled.  Without this monthly expulsion, the risk of uterine cancer increases. Women with evidence of hyperandrogenism and disordered menses have many other medical issues that must be addressed. This disease is reported to afflict 350-500 million people each year and causes over one million deaths.  Malaria causes a high fever, joint pain, shivering, vomiting and anemia.  If left untreated, it can result in severe medical complications or even death.  Unfortunately, there are no vaccines against malaria though they are being researched and developed for testing.<br />
The best treatment for malaria is to prevent it.  Some countries have a higher risk of infection than others.  There is no risk of malaria in North America and Western Europe.  All other areas have varying degrees of risk.  If traveling to a high risk area, it is advisable to seek consultation with a physician for preventative medications.  It is also recommended that insect repellants and mosquito netting be used to reduce exposure risk.  Because the mosquito has a high rate of reproduction and can become immune to medications quickly, seek a physician&#8217;s advice before using any oral medications.<br />
Treating malaria is much more effective when diagnosis occurs early.  Treatment in the early stages is much less invasive and generally has milder reactions.  If caught early, treatment can generally be accomplished at home under the supervision of a physician.  Only in severe cases or cases where diagnosis did not occur fast enough will hospitalization be required.  If traveling to a high risk country, be aware of the warning signs and get examined immediately.<br />
Minor cases of malaria are generally treated with oral medications.  It is also very important to ensure continual fluid intake so the body does not get dehydrated.  Quinine sulphate, along with doxycycline, is taken orally for one week.  There can be short term uncomfortable effects of this drug but its proven effectiveness in treating malaria is worth the temporary discomfort.  Malarone is a combination drug that is effective but very expensive and there have been reported cases of resistance to this treatment.<br />
More severe cases of malaria will require hospitalization for treatment.  Constant supervision by a physician is needed to ensure correct dosage and to monitor body responses.  Quinine is typically the medication provided for treatment.  Medications are administered intravenously in small doses because quinine taken in too large a dose can cause complications or death.<br />
Malaria is a very serious disease that is often mistaken for the flu.  If travelling to high risk areas or if you have recently returned from travelling, please seek medical attention immediately if you have flu-like symptoms.  Inform the physician that you have a risk of exposure to malaria.  Prevention and early detection are the keys to making certain malaria is treated and cured. </p>

<p>Tuberculosis was named after the Latin word tubercula, first in 1839.  tubercula means a small lump, referring to the scars that appear on the infected person&#8217;s skin. &#13;</p>
<p>In the 19th century, tuberculosis reappeared as a pandemic in Europe and US, determining people to research for the causes and possible treatment for TB. &#13;</p>
<p>Gaspard Bayle, a French physician described the lesions caused by TB in 900 autopsies.  &#13;</p>
<p>The evolution of TB from the first tubercle to the final stages was described by René-Théophile-Hyacinthe Laënnec. &#13;</p>
<p>An American physician, Edward Trudeau, was affected twice of tuberculosis, in different years, and he went to the mountain to spend his last days there, because he thought he was going to die.  There his symptoms ameliorated and he believed that the fresh air was responsible for his cure.  In 1885 he built the first American sanatorium which became a model for the other 600 sanatoriums that were built in America by 1930.  After that, Trudeau built the Trudeau Laboratory, which trained physicians in treating tuberculosis. &#13;</p>
<p>At first tuberculosis affected artists and morally superior individuals, but as the epidemic was spreading continuously, more and more people got affected including poor and disadvantaged people, and so, TB was thought to be related to a person&#8217;s lifestyle. &#13;</p>
<p>In 1882, Robert Koch, a German physician discovered the germ that caused tuberculosis and how the disease was transmitted. &#13;</p>
<p>In 1924, at the Pasteur Institute, in Paris, Albert Calmette and Camille Guerin used a virulent strain of bovine tubercle bacillus to prepare the BCG vaccine that was discovered to be highly efficient in preventing the disease. &#13;</p>
<p>Also, the beginning of modern anti-biotherapy for TB was instituted by Selman Waksman in 1944, when he isolated streptomycin from a fungus.  The antibiotic therapy became the primary treatment and so, mortality rates of TB decreased. &#13;</p>
<p>In 1904 in US there were188 deaths per 100000 and in 1980 were 1 per 100000. &#13;</p>
<p>This improvement lead to a cut in funding public programs for tuberculosis in the 1980&#8217;s, leading to a small rise of the number of persons affected of TB.  After that, TB continued to spread until the governments decided that tuberculosis was still needed to be paid attention, and refunded the public programs.  &#13;</p>

It is not necessary that all the infections are sexually transmitted so you should not assume that vaginal discharge is always an STD.  </p>
<p>Irritation of the vagina also known as vaginitis is the most common reason for discharges and is usually caused by infection.  There are mainly three types of vaginal infections and these all could be treated with oral or vaginal medications.  Such infection tends to produce a distinct discharge: </p>
<p> 1.    Usually there is a thick, white cottage cheese like discharge that is itching, irritated skin, yeast infection or candidiasis.  Women those who suffer from diabetes and those who take antibiotics are more prone to develop such infection.  Most of the women face at least one yeast infection at some point in their lives.  </p>
<p>  2.    There is a thin, yellow, foul smelling discharge known as trichomonas that is also transmitted sexually.  </p>
<p>  3.    Other symptoms include thin, gray or white foul smelling discharge known as bacterial vaginosis. </p>
<p>Pelvic inflammatory disease is frequently caused STD that infects the cervix, uterus, ovaries or fallopian tubes and is one of the most common and serious complications of an STD.  Symptoms include vaginal discharge or bleeding with lower abdominal pain and fever.  Chronic PID could result into one or more infections and the most common symptoms that could be identified are gonorrhea or Chlamydia that are sexually transmitted.  </p>
<p>Genital Herpes can also produce vaginal discharge and it could affect the cervix.  It includes features such as fever, itching, headache and general muscle aches.  Some women also get infection inside the uterus and this condition is known as endometriosis causing fibroid tumors, cancer or STD&#8217;s. </p>
<p>Sometimes a hole develops in the vagina and because of the passageway caused stool or urine passes through the vagina.  This problem can develop after a surgery or injury in the area, infection, inflammation or radiation. </p>
<p>Inflammation in the vagina is also caused because of lack of estrogen and as a woman enters into menopause her body produces increasing erratic amount of estrogen.  This often makes the vagina dry out and get irritated.  This condition is also known as atrophic vaginitis and could be treated by estrogen replacement therapy, vaginal creams or vaginal suppositories. </p>
